To register for online services, please complete a Basic Access Registration Form and bring it to the surgery reception, along with proof of identity. Two forms of proof are required:

• A birth certificate, driving licence or passport to prove your identity

• A household bill or bank statement showing your current address (dated within the last 3 months)

The registration form can be obtained from reception or downloaded here.

Once we have your completed application and have verified your identity, you will receive a letter containing your access codes and instructions to register with Patient Access.​
